Fed up with being Elizabeth, Sue resolves never to transform again when her seven-day period ends. Instead, she depletes all of her older self’s spinal fluid by draining it until there is none left. Forced to return due to lack of stabilizer fluid, Sue finds herself switching back. Since Sue disregarded the equilibrium and took everything from Elizabeth, leaving her permanently altered, distorted, hunched over, and resembling a crone.

In this situation, Elisabeth finds herself in distress and reaches out to The Substance hotline for guidance on ending the experiment. They provide her with an injection meant to terminate it, but Elisabeth hesitates to fully eliminate Sue. Instead, she halts before administering the entire dose and revives her alternate self instead. This unintended action leads to a strange glitch that keeps both women conscious at the same time. When Sue becomes aware of Elisabeth’s intentions, she lashes out. The confrontation between them is prolonged, gruesome, and brutal beyond belief. In the end, Sue overpowers Elisabeth, viciously beating her body into a bloody mess.

Since Elisabeth, only Sue remains as their shared self, but without additional support, her body also starts to decay. On New Year’s Eve, Sue dons a lovely blue dress for the show, but her body unexpectedly rebels. Her teeth loosen and an ear detaches. It soon becomes clear that she cannot perform in this condition on stage.

In a frantic search, she rushes back to the apartment to find the remaining activation serum that sparked everything, hoping to produce another improved version of herself called “Sue.” However, instead of creating a superior “Sue,” a grotesque creature, Monstro Elisasue, emerges from Sue’s body. This monstrosity is a fusion of Sue, Elizabeth, and various other body parts. Despite its deformed appearance, for the first time in the movie, this new entity, now known as “Elisasue,” seems content. In essence, she has discarded her old self and emerged anew.

Elisabeth re-donned her dress and stepped towards the New Year’s Eve stage, revealing her transformed self to the crowd, causing shock among the spectators. Her face could be seen peeking from her back, trapped and struggling to breathe.

In a shocking and terrifying turn of events, the spectators are sent into a frenzy upon witnessing Monstro Elisasue. The scene takes an even more gruesome turn when a new breast appears on her face, only to rupture and fall to the ground, dangling from a gushing vein. In response to this bizarre development, the audience attempts to assault and eliminate Elisasue, resulting in a brutal fight that causes her limbs to be torn off. The climactic event of this gruesome spectacle is Elisasue’s head exploding, creating one of the most outlandish sequences within the entire film as she showers the theater with an unending torrent of blood. The producers had joked earlier about wishing her breasts were in the center of her face!

Following the gruesome scene, Elisasue exits the theater, barely managing to do so. Her body begins to disintegrate, transforming into a pool of blood, organs, and other bodily fluids until only Elizabeth’s face is left amidst a pile of mud. With her final ounce of energy, Elizabeth moves towards the Hollywood Walk of Fame, eventually collapsing on her Elisabeth Sparkle star, mirroring the film’s opening scene. Slowly, she merges with the cement, and eventually, a street sweeper cleans her away permanently.

Regarding the large amount of blood shown at the end (specifically, 35,000 gallons), the director explains that this scene was essentially designed as a powerful statement of defiance against societal norms. The entire film serves to critically examine our culture’s fixation on unattainable beauty ideals and the way women are often marginalized as they age.
