It turns out he’s secretly terrified of blood, and the show brilliantly reveals it stems from a really painful experience. He was involved in a difficult surgery on a young girl that brought back all the grief from losing his sister, Rosemary, when he was a kid. It’s a surprisingly vulnerable backstory for someone who initially seems so tough.
The experience led him to seek a less stressful job: becoming the town doctor in a small Maine town where he used to spend summers with his aunt. Taking the position in Port Wynn meant he could still practice medicine while also dealing with his fear of blood.

Unlike many sitcoms that rely on general jokes, or serious medical shows with sharp wit, Best Medicine focuses on funny situations that come from its characters. A lot of the comedy happens when the straightforward Dr. Best interacts with the overly friendly people in town, creating moments that are both awkward and heartwarming.
The town of Port Wynn is a major reason the show has become so popular. It has a warm, inviting feel, reminding many of the charming community in Gilmore Girls. Port Wynn is filled with unique traditions, like the Blueberry Festival and monthly bean dinners, and even quirky spots like the Blood Factory. While it’s a bit of an idealized place, it feels incredibly welcoming.
This situation perfectly casts Dr. Best as an outsider. Similar to the character Gregory House from the show House, he notices everything and can’t overlook even the smallest medical detail. What To Expect From Best Medicine Season 2

If Best Medicine stays true to the style of the show it’s based on, the second season will probably explore Dr. Best’s fear of blood in more detail. Like in Doc Martin, where the doctor’s condition became common knowledge and a continuing part of the story, Dr. Best’s phobia could become a long-term challenge he struggles with.
The show could also explore new relationships within Dr. Best’s family. Like in Doc Martin, where the death of an aunt brought a new relative to town and changed the doctor’s life, a similar event could happen here. This would add depth to Dr. Best’s personal life, although the unique mix of warmth and criticism provided by Aunt Sarah would be hard to replace.
Meanwhile, Best Medicine will probably keep developing the unique atmosphere of Port Wynn. Because American TV seasons are usually longer than British ones, the show has more opportunity to delve into the town’s customs and everyday life.
Season 2 of Best Medicine will likely continue storylines we’ve already seen, while also starting new ones. This will allow the town’s unique traditions to change and develop as Dr. Best becomes more involved with the people who live there.
