The story begins with a difficult phone call. Liz, a nurse, finds herself in the unimaginable position of having to report her close friend, Jess, to social services. Jess arrives at the emergency room with her baby, Betsy, who has an unexplained head injury, and Liz is obligated to act. This event sets off the plot of the new Paramount+ series, Little Disasters, which is based on Sarah Vaughan’s novel. It quickly unravels the seemingly perfect life Jess has created with her husband, Ed, and their three children, and puts a decade-long friendship between Jess, Liz, and their friends Charlotte and Mel in jeopardy.

The final episode of Little Disasters begins with Jess facing potential jail time and a severe mental health crisis, leaving her on the verge of losing everything. TIME spoke with Kruger, Vaughan, and co-writer Amanda Duke about the episode’s surprising conclusion.

Honestly, while I’m still reeling from trying to figure out what happened to Betsy, what’s really upsetting me is how things fell apart between Jess and Liz. Liz reporting Jess to social services was a huge deal, and it’s completely turned Jess’s life upside down – she’s now facing a hearing to determine if she’ll even be able to see her kids again. I understand Liz had to follow hospital rules and make that call, and the show’s creator even said it was protocol. But even knowing that doesn’t make it any easier to see the impact it’s had on Jess. It’s just… devastating to watch.

During the court hearing, Liz spoke strongly in support of Jess, telling the judge what a great mother she is. She even admitted she often asks Jess for parenting advice. Moved by Liz’s heartfelt testimony, the judge agreed to release Jess on bail, as long as she lives with Liz. The judge also allowed Jess to have two supervised visits with her children each week.

According to Kruger, Jess is experiencing a lot of different feelings in that moment, but she appreciates Liz. Kruger believes Jess would have done the same thing if their roles were reversed. Throughout the show, Jess learns to accept that she needs help, and seeing Liz, a true friend, step up – even with a difficult task – is both confusing and helpful. While things might have been different if Liz hadn’t made the call, Kruger thinks Liz ultimately helps Jess begin to heal by pushing her to confront her issues. With Liz supporting Jess when she needs it most, their friendship has a chance to grow stronger.

Who really hurt Betsy?

Jess later meets her children with Ed at the school’s summer fair. It could be the last time the women are all together, as Mel’s husband, Rob, is moving the family out of London. Because Rob manages their finances and owns their house, Mel can’t prevent the move. During the fair, something unexpected happens: Mel’s son, Frankie, gets scared when he sees Rob and runs off.

Jess and her friends discovered him crying near a tree. When Ed passed the baby, Betsy, to Rob so he could comfort their son, Frankie suddenly yelled at Rob, telling him not to touch her. Through tears, Frankie explained that Rob was the one who had injured Betsy.

We learn through a memory that Rob visited the family home while Jess, exhausted from lack of sleep and struggling with postpartum OCD, had briefly gone out to get medication, leaving her young children unattended. Rob was hoping to discuss a business deal with Ed, who usually gets home early on Fridays but was still at work. Their son, Frankie, asked Rob to change his baby sister Betsy’s diaper, and Rob agreed. Frankie went upstairs to find a clean diaper, leaving Betsy on the changing pad on the kitchen counter. While Frankie was gone, Rob seized the chance to search the cabinets, finding one of Ed’s credit cards with the intention of forging his signature on a contract. However, during this, Betsy rolled off the counter and fell to the floor.

In this scene, Rob shows his cruel side and manipulates Frankie into believing he caused Betsy’s fall. After a brief check-up, he assures Frankie that Betsy isn’t seriously hurt, but warns that revealing what happened could get his mother in trouble for neglecting Betsy. Terrified, Frankie keeps the secret until he can’t bear it anymore and finally confesses at the fair.

The discovery completely shifts Jess’s perspective. Throughout the story, she’s struggled with the belief that her own stress and anxiety – and a form of obsessive-compulsive disorder experienced after having a child – had harmed her daughter. This guilt is made even worse by Rob allowing her to think she was to blame. According to the actor playing Jess, this moment is filled with anger towards Rob, as well as deep guilt over what her son Frankie has gone through. The show hints that Frankie is autistic, making Rob’s actions even more damaging. Despite everything, Jess also feels a sense of relief that she can finally reclaim her life.

The meaning behind the twist in Little Disasters

This change is a major departure from the source material. While Rob is the one who harms Betsy in the TV series, the book points to Charlotte, a friend of Jess’, as the responsible party. The book reveals Charlotte is consumed by envy over Jess’s seemingly perfect life and is also dealing with the heartbreak of failed IVF treatments. According to the author, Charlotte intends to confront Jess about how fortunate she is. Just like in the series, Jess is out running errands when Charlotte arrives. During a diaper change, a struggle ensues when Frankie pushes Charlotte, causing her to drop Betsy. In a moment of panic, Charlotte blames Frankie, mirroring Rob’s actions in the show.

According to Vaughan, the show’s creators suggested altering the storyline. She explains that the production company, RoughCut, didn’t want Charlotte to be portrayed as the one responsible because they thought it wasn’t believable. Vaughan points out a possible double standard regarding how male and female characters are treated, but ultimately, the producers struggled with the idea of a woman abandoning a harmed child.

Unlike Charlotte, who acted out of jealousy, Rob is driven by financial gain. According to Duke, Rob feels insecure about his earnings compared to other dads and families, constantly asking friends for money and seeking new contracts. He’s openly who he is – that annoying friend who always pushes boundaries. This behavior actually serves as a disguise for his truly awful actions, making it even more shocking when he starts to pressure Frankie. He’s incredibly destructive, and his behavior is unexpected because he initially seems like someone you can easily understand.

Duke explained they went through several versions of how to show Rob’s involvement in Betsy’s fall. Initially, they had a scene in the early part of Episode 6 where Frankie told Betsy she had fallen, but didn’t mention Rob being there. This would have shown the fall happen twice, which they worried would be too much for viewers. They ultimately decided to delay revealing Rob’s role because the early version made the later, more impactful reveal less effective.

Once Frankie reveals that Rob injured Betsy, Liz calls the police. According to Duke, this was a deliberate decision to create a sense of completion for Liz’s character. While there will be consequences, Liz consistently does what she believes is right, and Duke wanted to emphasize this full-circle moment for her.

In the end, Jess reunites with her life and children. A year after learning about Rob’s actions against Betsy, Jess and her friends are at the park with their families. However, Jess appears worried. She narrates, sharing that when things feel perfect, it’s important to cherish those moments, because happiness can change quickly. The scene closes with Jess holding Betsy, then fading to black.

Kruger explains that Jess understands how quickly things can change – a single lapse in attention can alter a person’s life. I think that applies to all of us, really.
