Be warned: this contains spoilers for the movie Keeper. Keeper is a deeply disturbing horror film that explores Malcolm’s past and reveals what happens to Liz. Directed by Osgood Perkins, the movie focuses on creating a chilling atmosphere, and much of the plot was kept under wraps before its release.
The movie Keeper features surprising twists that cleverly reinforce its central ideas, though the details of its supernatural elements are intentionally left unclear. This keeps the story focused on the characters Liz and Malcolm, and their ultimate destinies powerfully illustrate the dangers of extreme imbalances of power.
What Are The Creatures In Keeper, And Why Do They Want Liz?

The unsettling creatures haunting Malcolm’s cabin in Keeper aren’t fully explained, but their abilities and motivations make them particularly memorable and surprisingly sad. Throughout much of the story, the film deliberately keeps the true nature of these beings—those lurking around the house and within the woods—a mystery from the audience.
Late in the film, Malcolm explains some of the creatures’ origins. Centuries ago, a pregnant woman stumbled onto the family’s property and met Malcolm and Darren as children. Mistaking her for part of a religious group that repeatedly trespassed on their land, they injured and captured her.
While held captive, the woman gave birth to several strange beings. After Malcolm and Darren killed her, they made a deal with these creatures. Throughout the film, the creatures’ origins and what they truly are remain a mystery. What is known is that they can bargain with people – in this case, they extended Malcolm and Darren’s lives in exchange for sacrifices.
Interestingly, Malcolm confesses he doesn’t know what these creatures actually are. He can communicate with them, but we, the audience, never see how. This means we’re left to wonder about their origins and true nature, as the details remain a mystery.
Liz is in danger of becoming the next victim for the creatures, following a pattern of women being lured, cut off from help, and ultimately fed to them. The ghostly figures Liz sees are these past victims, foreshadowing the terrible fate that awaits her if she can’t escape the cabin.
Malcolm Is The True Villain Of Keeper

Although the strange creatures in Keeper seem like the biggest danger, the real villains are Malcolm and, to a lesser degree, Darren. They’ve been controlling women for centuries to stay alive forever. Throughout the movie, Malcolm pretends to be a kind and friendly doctor.
He seems kind and a bit clumsy, but that’s just an act. Despite claiming to love Liz, he’s willing to let her be killed by the creatures. In fact, he and Darren seem to enjoy these sacrifices – Darren even showed up with whiskey after Liz was almost killed, suggesting they were celebrating.
Malcolm embodies the idea of a controlling, emotionally detached man who’s willing to sacrifice others for his own gain. In the final scene, when Liz cries for help, he simply ignores her and turns up the music. But when his control is lost and the tables turn, his friendly facade disappears, and he resorts to desperate pleas and outbursts.
Malcolm embodies the quietly controlling and abusive type. This is powerfully shown through one of his creations, which bears the faces of all the women he’s harmed – a long list of ruined lives. While he appears gentle and harmless, he’s actually cruel, controlling, and far more terrifying than the monsters he uses to kill.
Why The Entities Spare Liz

Malcolm has pursued many women, but he chose Liz specifically because she looked like the creatures’ mother. He believed this resemblance would appeal to them, so he spent months carefully gaining Liz’s trust to guide her away from the city.
This decision turns out to be a significant error, but with an unexpected outcome. The creatures actually do recognize the similarity between Liz and their mother, even showing Liz a preserved head of her to demonstrate it. Surprisingly, this recognition leads them to become fiercely loyal to her. They consistently have chances to kill Liz, but each time they choose to spare her life.
Okay, so when Darren tries to hurt Liz, it’s actually the creatures who take him out – and it happens so fast she doesn’t even realize what’s happening! Then, instead of seeing her as prey when they corner her in the basement, they act almost…affectionately, like she’s a mother figure. Honestly, the connection between Liz and their mother is never really addressed, which is a bit frustrating. It leaves you with a lot of unanswered questions about why they react to her the way they do.
What really matters in the film isn’t the initial discovery itself, but what happens afterward. The creatures surround Liz, but surprisingly accept her as their mother, and she adapts to this new reality. This acceptance leads to the film’s ending, where both Liz and Malcolm are permanently transformed by the experience.
Liz And Malcolm’s Fates In Keeper, Explained

At the end of the movie Keeper, the otherworldly beings break their agreement with Malcolm and take him prisoner. This causes him to age very quickly. They leave him hanging upside down from a tree, where Liz finds him. Now, with eyes as dark as the creature’s mother’s, she tells him she’s chosen to remain with the entities.
Malcolm is screaming and begging, but Liz calmly tells him the cabin now belongs to her. She then forces him to eat the cake he used to drug her, and pushes his head into the sticky substance where the monsters had kept their mother’s head. This is a total flip from their earlier conversation, with Liz now coldly in control and Malcolm reduced to pleading.
The conclusion of Keeper hints that Malcolm will eventually be held accountable for what he’s done. While his crimes remain hidden, the women he harmed finally seem to have found some measure of peace. However, Liz’s ultimate destiny is less clear. It’s uncertain how much agency she still has, or whether she’s actually been reborn as a mother figure.
The True Meaning Of Keeper

As a film buff, I really appreciated how Keeper built this creeping sense of dread – it wasn’t about jump scares, but a constant feeling that something awful was lurking just out of sight. The movie does a fantastic job of teasing the monsters, letting your imagination run wild with their strange appearances. But honestly? The biggest twist is that the real threat isn’t some creature at all. It’s the seemingly friendly guy – that’s where the true danger lies, and it totally caught me off guard.
Honestly, the film really makes you think about how the creatures became so powerful – and so terrifying. It’s not just about their inherent nature, but the choices made by the characters. The film strongly suggests that the creatures’ monstrous evolution, and even their very existence in such numbers, is directly tied to the sacrifices – and specifically, the brutal act committed by Malcolm and Darren against their mother. It’s a haunting idea – that a little mercy could have prevented all of this, and spared everyone a far darker fate. It’s a key theme that really stuck with me after watching.
The film’s ending shows Malcolm shouting at Liz simply because she’s on what he considers his property. This highlights his old-fashioned belief that men should control everything, and his sense of ownership stems from a desire to feel connected to his family’s past. He also sees women as a means to achieve a kind of lasting legacy. Ultimately, Malcolm behaves like a spoiled child, and those around him treat him that way.
The creatures in Keeper aren’t villains; they’re fiercely protective of Liz, whom they consider their mother. Liz, however, mistakenly believed Malcolm was a good person, a belief that was tragically proven wrong. The story reveals that the real monsters in Keeper aren’t the fantastical beings, but the men who exploit and harm women to benefit themselves.
